  1. Workshop Request
This form allows faculty to request workshops on how to build either Basic or Intermediate ePortfolios. Please fill out the online request form here or fill out the word document and submit it to Sada Hye-Jaman via email at sjaman@lagcc.cuny.edu or drop it in her mailbox at M414.
  2. ePortfolio Workshop Feedback
This form is for instructors to rate the effectiveness of the ePortfolio workshop. Click here
  3. STM Request Form
This form allows faculty to request a Student Technology Mentor(STM) that can provide help with various technologies in the classroom or on a one-to-one basis. Click here
 RELEASE & ASSESSMENT FORMS
  4. ePortfolio Release Form
Students will sign the release form to allow the faculty member and the LaGuardia Center for Teaching and Learning to share their ePortfolios in professional venues to demonstrate the work of LaGuardia students and advance a broader understanding of the role of ePortfolios in teaching and learning.   5. Assessment Consent Form
By signing the Assessment Consent Form, students give permission to the members of LaGuardia Assessment Committee who will view samples of class work that students place in the Program Assessment folder in the ePortfolio system. This work will NOT be used to make judgments about the students and their individual progress. Instead, the college will use their work, anonymously, to better understand what students learn at the college and how the college can help them learn better. Click here
